Comments: This is by far the youngest male facelift patient of mine. He had premature facial skin laxity and jowls. He wished to look much younger if possible. To highlight the jawline, I recommended a square-chin implant, in combination with liposuction, a facelift, and trimming the buccal (cheek) fat pads, which is done using incisions on the inside of the mouth. This is used to treat the “chipmunk” look. Afterward, he thought he looked a decade younger and received compliments from friends and family. His significant other thought he looked “fabulous.”

Comments: This 45 year-old male was disappointed after having a facelift elsewhere two years previously. The original surgeon had re-operated without appreciable benefit. His earlobes were distracted downward and he had scar tissue of the right jawline. His facelift was redone, taking care to elevate the deep tissues and avoid any skin tension. The patient elected to have his eyelids treated simultaneously. A jowl implant was used to further accentuate his jawline. The Sclerolaser was used to treat redness of the neck. This case illustrates the importance of elevating the deep tissues and avoiding skin tension to avoid an “operated on” look after surgery.

Comments: This patient was interested in looking younger and better for personal and professional reasons. He had old acne scars of the chin and depigmentation from a previous laser treatment done years ago in another office. These procedures were done in stages. A square-chin implant was used and the buccal fat pads were trimmed to correct the “pudgy look”. A tip rhinoplasty was performed to correct his bulbous tip. Afterward, he felt more confident in the youth-conscious medical sales business. He said some fortysomethings thought he was their contemporary, even though he’s a decade older. He was concerned before surgery that he might look “operated on” afterward and relieved that he did not.

Comments: Having retired early, this patient was returning to the work force. He wanted to look younger to be able to compete in a youth-conscious computer industry. The idea of a stronger, masculine jawline appealed to him. He was a smoker but healed well. He did have a hematoma after surgery which was treated promptly. He thought he looked ten to twelve years younger. He had some persistent hyperpigmentation of the lower lids which was treated with a bleaching cream.

Comments: This 58 year-old banker and hotrod enthusiast wanted to look younger. His wife was about forty. He thought his eyelids made him look older. He understood that his low eyebrow position contributed to his droopy eyelids. Also, he wanted something done about his “turkey neck”. His longstanding nasal obstruction was relieved simultaneously. He returned to work after only two weeks off. His wife presented for surgery the next year.

Comments: This 59 year-old physician had previous blepharoplasties and a “mini-facelift.” He requested liposuction of his mid-section simultaneously. His old facelift scar was located a little too far in front of his ear, making it more noticeable. Men, of course, cannot hide this scar as easily as women. After his facelift, the scar is better concealed in the crease in front of his ear. He said the recovery was pain-free, except for the first night after surgery, when the dressing was uncomfortable. Later, he had laser hair removal of the areas behind the ears (the hair-bearing area gets lifted in men). He was particularly pleased with his neckline and the improvement in facial wrinkles.

Comments: This 61 year-old was looking forward to retirement. He was an avid windsurfer and wore an ear ring. He is married to a woman who is about twenty years his junior. He articulated his concerns well – the bagginess of his eyelids, loosening of facial skin with jowls, facial wrinkles, and the skin laxity of his neck. Indeed, he had virtually no jawline on his sideview. The Sclerolaser™ treatment had reduced his facial redness and he said he no longer became as flushed in the face if he had a glass of wine. We discussed a possible touch-up laser treatment and fat injection for the persistent wrinkles around the chin, but he has not scheduled this yet.

Comments: This patient has a long history of sun exposure, an admitted past sun worshipper. He has a fair complexion. Note the extensive sun-induced brown spots that have been improved by the laser treatment.
